Each year the chapter must submit a request to the 375th Support Group CC for approval to operate a private organization on Scott AFB. The 375 FSS documentation for this requirment is available here.
The approval package must contain the following information:
- AF Form 1768, Staff Summary Sheet
- Quarterly Financial Report (Income/Expense/Balance Sheet)
- List of Current Officers
- Treasurer Acceptance Statement
- Current Financial Audit Report
- Current Year Budget
- Waiver of Liability Insurance
- Insurance for President/Treasurer/Secretary
- By-Laws (Obtain latest copy from AFSA website)
- Policies and Procedures (Obtain latest copy from AFSA website)
Once the package is compiled and properly tabbed, it can be submitted to the 375 FSS for staffing and approval. It is important that this process be started NLT 1 July each year to ensure it is approved by the end of the month. If this is not accomplished the chapter can not operate on Scott AFB.
